The acute fatty liver of pregnancy is a pathology of second half 01 pregnancy, rare, but with an elevate maternal mortality. The best fetal and maternal prognosis is obtained with pregnancy interruption and support treatment, avoiding complications of this entity: renal failure, hemorrhage, coagulopathy, hypertensive crisis, hepatic failure, pulmonary edema, encephalopathy, coma and death. We present a 38 years old female, G3P0C2A0, with 35 6/7 weeks pregnancy, with fever, headache and vomiting, two days ago. Furthermore, hypertension (150/100) and alterations in laboratory test results, that suggest hepatopathy and defective coagulation results, with fever, anorexia, abdominal pain and jaundice. The patient underwent a cesarian surgery and partial bilateral salpingectomy, obtaining a 2280 g male product, APGAR 7/9, presenting hypotension during the surgery and later defective coagulation pathology.
