MULTIPLE SCLEROSIS:: A CASE REPORT

Abstract

A 31 year-old male was admitted with objective vertigo and gait disturbance of one month of duration. The magnetic resonance image showed supratentorial and infratentorial gadolinium-enhaced lesions. The symptoms got worse later, developing a Bell’s palsy and a right internuclear ophthalmoplegia. Visual evoked response came abnormal, the auditive’s, normal. Anticardiolipin IgM antibodies became strongly positive, but in a second intent, they resulted  negative. Cerebrovascular disease of young adults and thrombophilia were rejected because of negative results on tests like Protein C, C3, C4, Antithrombin III. The treatment with methylprednisolone made him feel better. There are still pending the laboratory results for oligoclonal bands in cerebrospinal luid and space and time results of a new magnetic resonance. Dissemination of lesions in space of the McDonald criteria were fulilled and it was concluded that the definitive diagnosis was Multiple Sclerosis. The discharged treatment was ?-interferon 1b with diphenhydramine and acetaminophen.
