INTRODUCTION. The prevalence of atherosclerotic left main coronary
artery disease diagnosed by angiography can vary from 1 - 3.6%.
OBJECTIVES. To determine the prevalence of atherosclerotic left main coronary artery disease in patients with ischemic heart disease treated at the Hemodynamic Laboratory of the Complejo Hospitalario Metropolitano Dr. Arnulfo Arias Madrid.
MATERIALS AND METHODS. We performed a retrospective observational study. We analyzed the records of patients with ischemic heart disease treated at the Hemodynamic Laboratory of the Complejo Hospitalario Metropolitano Dr. Arnulfo Arias Madrid, from April to December of 2008. The study universe comprised 439 patients, we studied the entire universe. We calculated the prevalence of atherosclerotic left main coronary artery disease, made a description of this population and compared it with the patients with ischemic heart disease according to demographic and clinical variables.
RESULTS. We identified 30 patients. The prevalence of atherosclerotic left main coronary artery disease was 8.2%. There was a significantly higher prevalence of multivessel coronary artery disease, trivascular associated coronary artery disease and anterior descending coronary artery disease, in the patients with atherosclerotic left main coronary artery disease when they were compared to patients with ischemic heart disease (p = 0.001, p = 0.001 and p = 0.001). There was a marked impairment of myocardial contractility (61.2%) and high prevalence of non-preserved ejection fraction (56.6%). Surgical management was the most common recommendation of treatment (66.7%).
CONCLUSIONS. The prevalence of atherosclerotic left main coronary artery disease was 8.2%.
