Abstract

Ankylosing spondilitys, an uncommon reumathologic entity, is an inflammatory arthritis ot the spine that involves the sacroiliac joints.  It frequently also affects peripheral articulations, and these patients need mayor orthopaedic surgeries such as total knee replacement.

This case is about a young woman with advanced spondylitis, who required this quirurgic procedure and therefore special anesthetic intervention. The preanesthetic evaluation showed true predictors ot difficulty in the management of the airw ay and app licalion of reg ional anesthesia techniques. She was unable to lie prone. The pro cedure was performed under general anesthesia, afler int uba tion with fibroscopy because of the peculiarity of her airway. The apportunity is taken to discuss features and camplica fions of the disease which can lead to pe rioperalive difficulties, the importance of a pre anaesthetic evaluation and the proposed modalities tor the anesthelic management of the se petients.
