ASYMPTOMATIC PELVIC MASS IN A 24 YEAR-OLD WOMAN. “THE BENIGN MONSTER”

Abstract

Introduction: Ovarian teratomas are germ cell neoplasms. Familiarizing the medical community with key aspects of these tumors is crucial due to their high incidence among young women and their potential impact on fertility.

Case Report: We present the case of a 24-year-old female with a history of exploratory laparotomy six years prior due to acute abdomen, where a twisted right adnexal mass was discovered. She underwent routine follow-up with control ultrasonograms. Six years later, an ultrasonogram detected a tumor in the left ovary, prompting her referral to the Instituto Oncológico Nacional. The tumor appeared as a round echogenic mass in the lower half of the ovary, with calcifications, and the ovary was also enlarged. The patient was asymptomatic and had no histopathologic report from her previous surgery. Physical examination revealed a mass in the left adnexa, an absent right adnexa, and a free cul-de-sac. Laparoscopy was performed, and the ovarian mass was excised intact, preserving the ovarian capsule. The tumor contained hair, and a sample was sent for pathology. Histopathologic analysis confirmed the diagnosis of a mature cystic teratoma.

Discussion: Diagnosing an adnexal tumor is challenging due to the variety of clinical conditions that can affect the ovary. In this case, ultrasound revealed calcifications, which supported the diagnosis before surgery.
