PULMONARY INFECTIONS IN HIV: Pneumocystis Carinii PNEUMONIA

Abstract

In HIV-infected patients, pulmonary complications occur frequently and are an important cause of morbility and mortality in these patients. In the beginning of the AIDS epidemic, Pneumocystis carinii pneumonia ocurred in 75% of infected patients. The incidence has diminished due to administration of prophylactic therapy.

We present a clinical case of a 33-year-old femenine patient, with history of HIV infection fse five years, who clinically presents diarrea, nausea, fever, productive cough and weight loss with a duration of 2 moaths. Upon fecal examination, parasitosis by Giardia lamblia and E. hystolitica is evidenced. A fibrabroncboscopy shows abundant purulent material anda bronco-alveolar wash is performed, demonstrating the presence of S. pyogenes and P. carinii.

It is important to note that a CD4 count of 213 cell/mm³ was obtained and the patient had never received treatment for her HIV infection. nor prophylaxis against P. carinii.

A bibliografic review on Pneumocystis carinii was carried out in order to discuss the patients findings and compare them to what the literature dictates.
